Perio forms are created in axiUm like other forms but can be set to display only in the Perio module, or shared so that they display in the Perio, EHR, and Ortho modules.

Once the perio form is created, working with perio form pages and status codes is the same as working with regular EPR forms. For more information, see Form Setup.

Note: All fields in the Perio Form Setup window are the same as the Form Setup window, except for the Form Type field, which is described below.

Field Description
Form Type

A drop-down list with the following options:

  • Perio EPR Form: Forms are only accessible in the Perio module and not from the EHR or Ortho modules.
  • Shared EPR Form: Forms can be viewed from Perio, EHR, and Ortho modules.
