Perio Answer Lists Window

Perio answer lists are created in axiUm like other form answer lists but can be set to be used only with perio forms, or shared so that they are available in Perio, EHR, and Ortho forms.

Notes:

  • All fields in the Perio Answer Lists window are the same as the EPR Answer Lists window, except for the Form Type field, which is described below.

  • Additionally, the EPR Answer Lists window has an Is ASA check box, that is not present in the Perio Answer Lists window.

Field Description
Type

A drop-down list with the following options:

  • Perio EPR Form: Answer lists are only accessible in the Perio module and not from the EHR or Ortho modules.
  • All: Forms can be viewed from Perio, EHR, and Ortho modules.
