EPR Answer Lists Window

The EPR Answer List window allows you to configure the answer lists available for use within EPR forms.

Field Description
Code

A text field that indicates the code that identifies the answer list.
Description

A text field that describes the answer list.
Type

A drop-down list that indicates the type of notes that this answer can be used with:

  • Dental EPR: These answer lists can only be used for clinical template notes only.
  • All: These answer lists can be used for clinical template notes and EPR forms.

    • Note: When set to All, the answer lists are accessible from the EPR Forms Answer Lists, Perio Answer Lists, and Ortho Answer Lists windows.

Is ASA A checkbox that, when selected, indicates that responses (list items) in this list have an ASA (American Society of Anesthesiologists) score value.
Task Procedure
Add Answer List
  1. Click the Clear Data () icon.
  2. Complete the fields as necessary and click the Add a new Record () icon.
Specify Answer List Items
  1. Select a listed entry and click the List Items() icon to display the List Items window.
  2. Configure the list items as required and close the window.
Edit Answer Lists
  1. Select a listed entry.
  2. Make any necessary changes and click the Modify Record () icon.
Delete Answer Lists
  1. Select a listed entry and click the Delete Record () icon.
  2. When prompted, click Yes to confirm the deletion.
Preview/Print Report
  1. (Optional) To preview the EPR Answer Lists report, click the Preview Report () icon.
  2. To print the EPR Answer Lists report, click the Print Report () icon.

    3. Note: The EPR Answer Lists reports uses the lists.rpt file.

List Items Window

[Accessed from Maintenance > EHR > EPR Forms Answer Lists > List Items () icon]

The List Items window allows you to configure the list of available answers that can be selected for this answer list. It displays when you open the EPR Answer Lists window, select a listed answer list and click the List Items () icon.

Field Description
List Code / List Name

Read-only fields that indicate the selected answer list.
Item Text

The text that displays in the list.
Reference Code (optional)

A text field that indicates the reference code for the answer.

Note: This field is used for imports and reporting.
ASA Score

A drop-down list that indicates the ASA score to associate with this answer:

  • N/A
  • 1
  • 2
  • 3
  • 4
  • 5

    • Note: This field is only enabled if the Is ASA checkbox is selected in the answer list.

Task Procedure
Add List Items
  1. Click the Clear Data () icon.
  2. Complete the fields as necessary and click the Add a new Record () icon.
Edit List Items
  1. Select a listed entry.
  2. Make any necessary changes and click the Modify Record () icon.
Delete List Items
  1. Select a listed entry and click the Delete Record () icon.
  2. When prompted, click Yes to confirm the deletion.
Reorder List Items
  1. Select a listed entry.
  2. Click the Up and Down buttons to move the selected entry within the list.
