Ortho Form Setup Window

Ortho forms are created in axiUm like other forms but can be set to display only in the Ortho Manager module, or shared so that they display in the Ortho, EHR, and Perio modules.

Once the ortho form is created, working with ortho form pages and status codes is the same as working with regular EPR forms. For more information, see Form Setup.

Note: All fields in the Ortho Form Setup window are the same as the Form Setup window, except for the Form Type field, which is described below.

Field Description
Form Type

A drop-down list with the following options:

  • Ortho EPR Form: Forms are only accessible in the Ortho module and not from the EHR or Perio modules.
  • Shared EPR Form: Forms can be viewed from Ortho, EHR, and Perio modules.
