Facilities Window

Facility codes are used by US medical insurance companies to indicate the facility address where a service was provided.

The facility information prints on both the HCFA and ADA forms.

On the original HCFA, HCFA 2006, and HCFA 1500 forms, the facility name and address prints in box 32.

On the ADA 2006 and ADA 2002 forms, the facility address prints in box 56.

On the ADA 2000 form, the facility address prints in boxes 63, 64, 65, and 66. No facility names are printed, only the facility address.

The Facilities window allows you to manage the list of facilities and specify the list of services offered.

Field Description
Code

A text field that indicates the code that identifies the facility.
Description

A text field that describes the facility or department.
Other Location?

A check box that, when selected, indicates that this facility is not the home or office facility indicated in box 33 of the HCFA claim form.

When selected, box 56 of the ADA form will be filled in with this address, otherwise box 56 will be blank.

For EDI claim form processing, axiUm includes a separate segment.
Inactive

A check box that, when selected, indicates the parent company is inactive and not in use.
Address

Text fields that indicate the address or location information for the facility.

The ellipses are used to display the City List window and State Codes window and select a city and state.
SSN / TIN

Radio buttons that indicate the type of license number (Social Security Number or Tax Information Number) and a text field that indicates the full license number.
NPI

A text field that indicates the personal NPI number to print on insurance claims generated for this billing doctor.

This is a 10-digit numeric code.
Note A text field that indicates any comments relevant to this facility.
Task Procedure
Add Facilities
  1. Click the Clear Data () icon.
  2. Complete the fields as necessary and click the Add a new Record () icon.
Specify Facility Services
  1. Select a listed entry and click the Facility Services () icon to display the Facility Services window.
  2. Manage the list as necessary.
Edit Facilities
  1. Select a listed entry.
  2. Make any necessary changes and click the Modify Record () icon.
Delete Facilities
  1. Select a listed entry and click the Delete Record () icon.
  2. When prompted, click Yes to confirm the deletion.
Preview/Print Report
  1. (Optional) To preview the Facilities report, click the Preview Report () icon.
  2. To print the Facilities report, click the Print Report () icon.

    3. Note: The Facilities report uses the facility.rpt file.

Facility Services Window

(Maintenance > Insurance > Facilities Codes)

The Facility Services window allows you to configure the list of services available at the facility with a list of service type codes. It displays when you open the Facilities window, select a listed facility and click the Facility Services () icon.

Field Description
Code

A disabled field that indicates the selected facility.
Service

A text field that indicates the service type.

The ellipsis is used to display the Service Type Codes window and select a type of service.
Number

A text field that indicates the service number to use for this facility when a claim is sent to an insurance company.

Tip: This can be any number required by the insurer such as a Medicaid number or facility ID number.

A drop-down list that indicates the type of license number.

Note: The options in this drop-down field are hard-coded in axiUm and are not user-defined.
Task Procedure
Add Facility Services
  1. Click the Clear Data () icon.
  2. Complete the fields as necessary and click the Add a new Record () icon.
Edit Facility Services
  1. Select a listed entry.
  2. Make any necessary changes and click the Modify Record () icon.
Delete Facility Services
  1. Select a listed entry and click the Delete Record () icon.

  2. When prompted, click Yes to confirm the deletion.
