A drop-down list that indicates the type/location of the field in axiUm:

Adjustment 1-2:Up to two fields that are displayed in the Transactions module > Adjustment tab.

InfoMgr Patient Date 1-10:Up to 10 custom dates that are used in the Info Manager module.
Note: Info Manager Patient Dates require a custom oracle view to be created.
- Insurance Payment 1-2:Up to two fields that are displayed in the Transactions module > Ins Pmt tab.
- Patient Payment 1-2:Up to two fields that are displayed in the Transactions module > Payments tab.

Patient 1-10:Up to 10 fields that display in the Patient Info window > Codes tab.

Patient Text 1-5: Up to 10 fields that display in the Patient Info window > Codes tab.

Prescription 1-2: Up to two fields that display when entering prescriptions in the EHR module.
- Treatment (Clinical) 1-2:Up to two fields that display when entering treatment via the EHR and Treatment Planning modules.
- Treatment / Note 1-2:Up to two fields that display when entering treatment via the EHR and Transactions modules.
- User 1-10:Up to 10 fields that display in the Users window > Personal tab.

Description

A text field that indicates the label to display for the custom field.

Validate using Custom Code table
|
A check box that, when selected, indicates that this field will display a drop-down list of available answers, as configured in the Custom Codes window.
When deselected, the field uses free text.
Note: This field is disabled when Field is set to a InfoMgr Patient Date or Patient Text entry. Additionally, this field is not considered for User fields as they must use the Custom Code table, regardless of the check box status.

Entry is required

A check box that, when selected, indicates that users must complete this field.
Note: This field is disabled when Field is set to one of the InfoMgr Patient Datefields.

Is the Tx Location field (used in Department definitions)
|
A check box that, when selected, indicates that this field is used in Department definitions.
Note: This field is only displayed when Field is set to one of theTreatment/Note entries and can only be selected once.
