Survey Names Window

The Survey Names window allows you to configure surveys for use by your institution.

Field Description
Code

A text field that indicates the code that identifies the survey.
Description A text field that describes the survey.
Student

A check box that, when selected, indicates that the student's signature is required when users print the survey.
Instructor A check box that, when selected, indicates that the instructor's signature is required when users print the survey.
Patient A check box that, when selected, indicates that the patient's signature is required when users print the survey.
Task Procedure
Add Survey Names
  1. Complete the fields as necessary and click Add.
Edit Survey Names
  1. Select a listed entry.
  2. Make any necessary changes and click Modify.
Delete Survey Names
  1. Select a listed entry and click Delete.
