View Perio Form History

Select a patient and open the Perio Chart module. Click the EHR Forms () icon to display the Perio EHR Forms window. Note: If the patient does not have any perio exams on file yet, the fields are blank. Select an entry from the Form drop-down and click the Show History icon () to display the Select Form History window. Select a listed date and click OK to display the View Form History window. Review as necessary.
