Preview / Print Perio Forms

  1. Select a patient and open the Perio Chart module.
  2. Click the EHR Forms () icon to display the Perio EHR Forms window.

    3. Note: If the patient does not have any perio exams on file yet, the fields are blank.

  3. Select a form and click the Print Report () icon to display the Print Form window.

  4. Select a radio button:

    • Standard Reports: This prints the form as it appears in the Perio Chart module.

      • Note: The standard Perio Form report uses the PtForm.rpt file.

    • Custom Reports: This uses a customized Crystal Report specific to the form.
  5. If you selected the Custom Report radio button, select an option from the drop-down list.
  6. (Optional) Click Preview to view the form.
  7. Click Print.
